Medical and dental errors result in an unacceptable level of risk for the patients treated in hospitals as well as in ambulatory outpatient care facilities. In 1999, the Institute of Medicine revealed in its report “To Err is Human: Building a Safer Health System” that up to 98,000 death occurred in USA due to preventable medical errors.
